    I’m in my mid twenties and have had weak erections for the past few years. Very little empathy from doctors given my age and ongoing autoimmune issues over the years. Would like to see you and also have your opinion on the following issues if they are a concern: 1) only partial engorgement during carveject assisted ultrasound 2) elevated e2 on some of my blood draws 3) experimenting with alpha-blockers give me much, much better erections then pde5s alone.

    Also very random but vaguely related to andrology: do bimatoprost or minoxidil have any effect on facial hair growth?

    • Interesting. You probably have a venous leak. You say your E2 is elevated. That is not uncommon in someone who is mid-20s. More importantly, how elevated and what about the other hormones? Even more interesting alpha blockers give you a good erection even though they are supposed to cause ED as a side effect. Just off the top of my head – autonomic hyperactivity. Minoxidil – no evidence. Bimatoprost – can improve eyebrow growth so theoretically should improve facial hair growth but also no direct evidence. If you have little facial hair, you need to think about hormone related issues maybe also causing your ED.

    1. Do you believe that all generics are created equal (pertaining to PDE5s)? I’ve never heard of Remenefil. What pharma is that from? I’ve read anecdotes and have had experience with different generic brands which seem to vary in potency.

    2. Is it important to always do a morning draw for my hormone levels?

    3. Are weekly injections always the most optimal, and that monthly or gel/compounded creams/orals always due to low patient cooperation or physician inexperience?

    4. Is ESWT approved for elective treatment just like the machines in various beauty and wellness clinics?

    5. Lastly, will you be consulting in the new clinic?

    • 1. Very good question. Absolutely NOT. All generics are not created equal and they can never be as good as the original. We use a concept called bio-equivalence. Very few generics even come close to 90% bio-equivalence to the originals. Remenefil is marketed by Menarini. They are one of the bigger pharmaceutical companies. The product is manufactured in Europe. So far, I have not had bad feedback from my patients (knock on wood). But they are new in the market and I am adopting a wait-and-see approach. Although this sounds mercenary, if you can afford it, it’s better to stick to the patented stuff. 2. You are referring to Testosterone right? The recommendation is to draw the sample before 11am due to the diurnal variation in levels. However, the recommendation is also to take 3 separate samples to confirm the diagnosis. I personally don’t think there is a huge difference unless the results are borderline. 3. You mean which Testosterone replacement technique is the best? Everyone of them have their pros and cons. I have patients do well on injections, gels and pills. I think the therapy has to be customized to the patient’s lifestyle. Nobody I know does weekly injections anymore. 3 weekly maybe. 4. Woah man. The ESWT machine for treatment of ED is sold to doctors only and can only be operated by doctors. It is serious stuff. I would be extremely concerned if HSA allows beauty and wellness clinics to use it. 5. Yes I am at Novena full time.

  3. Hi, I would like to have some help. I want to adopt a child, 7 years old HIV positive. I am not Singaporean and I will be here for more 2 years. How to I get to treat the child? What type of support I can get from the government if I am not Singaporean? Thanks in advance. Karla

    • Good question. I assume you have already checked with MSF (Ministry of Social and Family Development) and gotten the requisite clearances. AS for treatment, you can bring the child to any private Infectious Disease specialist. Or you can bring the child to CDC Singapore. I honestly doubt you will get any kind of support from the government. Even Singaporeans with HIV get hardly any support so that should give you an idea of what you can expect.

    • I assume your prostatectomy was nerve sparing. The treatment of ED after prostatectomy is called penile rehabilitation. It is usually done with PDE5Is, Penile Injections, Vacuum Devices and Penile Implants. The role of ESWT in penile rehabilitation is not fully established but there is some evidence to say that it works. Personally, I would first trial a patient on PDE5Is and Penile injections before proceeding to ESWT.
